Posterior capsule opacification (PCO) following cataract surgery occurs due to viable lens epithelial cells from the anterior capsule migrating backwards behind the intraocular lens implant. The previously clear posterior capsule, supporting the implant, develops wrinkling or opacification (Elschnig’s pearls).
